A Case Management Assessment addresses the client's ongoing accident related needs, in order to assist in the development and execution of a sustainable and interdisciplinary rehabilitation program. The Case Management Assessment focuses on promoting quality of life through the maximization of independence and function at home and in the community. The assessment includes review and analysis of demographic information; medical history; employment history; educational background and; medical information to determine clinically indicated services. Interview with client and / or family is conducted within the client's environment to assess the client's current rehabilitation treatment and medical needs. A comprehensive report is prepared outlining innovative rehabilitation treatments to respond to identified needs while maintaining a progressive environment.
A Rehabilitation Assessment addresses the client's ongoing accident related needs, in order to assist in the development of a rehabilitation program. The assessment process will identify the client’s post injury rehabilitation needs and recommendations for optimal resource utilization. The assessment report provides a streamlined and refined rehabilitation plan with timelines and indicators for success.
The Rehabilitation Assessment is conducted within the client's environment to identify the client's current rehabilitation and medical needs focusing on rehabilitation goals to maximize home, community and work activities. Rehabilitation assessment provides coordination and communication of available services and community resources with client, family and all other health care providers.
A Case Manager will assist with the facilitation of rehabilitation services for adult and pediatric individuals who have sustained personal injuries. Services will include the development and execution of discharge and rehabilitation plans. Through open dialogue, the Case Manager will serve as a liaison between the client family and treating professionals to maximize communication within the rehabilitation team and to ensure an optimum level of service provision. The coordination of team meetings will be facilitated by the Case Manager to maintain continuity of programming by incorporating strategies as recommended by the various disciplines. With over twenty years of experience, innovative models of service delivery and resources will be accessed to assist in achieving desired clinical outcomes; improved client functioning and quality of life and; where possible a return to pre-accident roles, responsibilities, activities and interests.
The facilitation of the completion of the Catastrophic Designation Application Form (OCF 19) can be coordinated by the Case Manager. The Case Manager will contact and meet with the appropriate healthcare provider(s) to ensure completion and submission of the CAT application form.
A Case Manager will ensure that the client progresses through the continuum of care with a seamless transition from hospital to community. Collaboration with the client, family, health care team and community resources will promote optimal recovery. The Case Manager will provide comprehensive identification, assessment, planning, implementation, coordination and evaluation of individual client rehabilitation needs.
