Wheelchair Assessment
 

 


 

 

The purpose of this assessment is to thoroughly review the client’s physical, cognitive and affective accident related injuries, impairments and sequelae for the purpose of determining the impact they have on the client’s ability to perform functional wheelchair mobility in a safe and independent manner.

 

An Occupational Therapist will assess the client within their home environment to determine the most appropriate manual wheelchair to meet the client’s functional mobility needs.   The Occupational Therapist will complete a full physical and functional Assessment with the client.  The Physical assessment will include a full postural analysis to identify presenting postural deformities that require specialized seating supports for correction or accommodation.  An environmental assessment will also be completed to ensure the recommended manual wheelchair is accessible within the client’s home, work, school or other environments in which the client accesses on a regular basis.

 

Following completion of the assessment, recommendations will be made for the most appropriate walker device that will allow the client to work towards resuming their mobility in a safe and independent manner within the environments previously accessed prior to the accident.  An application for funding with the Ministry of Health Assistive Devices Program (ADP) may be completed on behalf of the client if they are assessed to be eligible for funding.

 

This assessment will include both qualitative and quantitative methods; including completion of a clinical interview, physical assessment, cognitive and affective screening protocols, functional assessment and a full postural analysis. A medical chart review will be completed if available.

 

·         File review, communication with client and or caregiver to determine current mobility status and liaison with client’s vendor of choice to request trial equipment for the assessment

·         Assessment of the client within their home environment to determine the most appropriate manual wheelchair and seating system to meet the client’s functional mobility and postural needs

·         Additional 1-2 assessment visits (2 hours each visit) to facilitate equipment trials

·         Complete full report of assessment findings and recommendations. If applicable, the Occupational Therapist who is a registered ADP authorizer will complete the appropriate application for funding assistance.

·         Completion of a follow up visit with the client at the time of the delivery of the recommended device will occur to ensure the appropriate fit of the device and to ensure it is meeting the identified postural and mobility goals.  During this follow up visit education and training of safe use of the manual wheelchair will be provided.

·         Additional liaison with client’s vendor of choice regarding equipment needs for trial and additional assessment visits.

·         Travel and mileage in addition to the above noted breakdown.

Power Wheelchair Assessment
 


An Occupational Therapist will assess the client within their home environment to determine the most appropriate power wheelchair including power tilt/recline or other alternate drive controls to meet the client’s independent and functional mobility needs. 

 The Occupational Therapist will complete a full physical and functional Assessment with the client.  The Physical assessment will include a full postural analysis to identify presenting postural deformities that require specialized seating supports for correction or accommodation.  An environmental assessment will also be completed to ensure the recommended power wheelchair is accessible within the client’s home, work, school or other environments in which the client accesses on a regular basis.

·         The Occupational Therapist will meet with the client and their vendor at the time of the delivery of the recommended power wheelchair and seating system to ensure appropriate fit of the equipment as well as education and training of safe use of the power wheelchair system.

·         Depending on the type of device being recommended for the client an additional 1-4 follow up visits (1.0 hour each) may be required for custom seating fabrication, mobility training for the client and their caregivers

·         Additional liaison with client’s vendor of choice regarding equipment needs for trial and additional assessment visits.

·         Travel and mileage in addition to the above noted breakdown.


 

Power Scooter Assessment
 


 

The purpose of this assessment is to thoroughly review the client’s physical, cognitive and affective accident related injuries, impairments and sequelae for the purpose of determining the impact they have on the client’s ability to perform their functional mobility in a safe and independent manner.

 

An Occupational Therapist will assess the client within their home environment and surrounding community to determine the most appropriate power scooter is required to meet the client’s functional mobility needs.   The Occupational Therapist will complete a full physical and functional Assessment with the client.  The Physical assessment will include a full postural analysis to identify presenting postural deformities that require specialized seating supports for correction or accommodation.  An environmental assessment will also be completed to ensure the recommended scooter has access into the client’s home, work, school or other environments in which the client accesses on a regular basis.

 

Following completion of the assessment, recommendations will be made for the most appropriate power scooter device that will allow the client to work towards resuming their functional mobility in a safe and independent manner within the environments previously accessed prior to the accident.  An application for funding with the Ministry of Health Assistive Devices Program (ADP) may be completed on behalf of the client if they are assessed to be eligible for funding.

Walker Assessment
 


The purpose of this assessment is to thoroughly review the client’s physical, cognitive and affective accident related injuries, impairments and sequelae for the purpose of determining the impact they have on the client’s safety and independence with their functional mobility using a standard or rollator walker.

 

An Occupational Therapist will assess the client within their home environment to determine the most appropriate mobility device to meet the client’s functional mobility needs.   The Occupational Therapist will complete a full physical and functional Assessment with the client.  The Physical assessment will include a postural and gait analysis to identify presenting musculoskeletal and cardiovascular impairments contributing to the client’s mobility limitations and to determine the most appropriate mobility device. An environmental assessment will also be completed to ensure the recommended walker is accessible within the client’s home, work, school or other environments in which the client accesses on a regular basis.

 

Following completion of the assessment, recommendations will be made for the most appropriate walker that will allow the client to work towards resuming their mobility in a safe and independent manner within the environments previously accessed prior to the accident.  An application for funding with the Ministry of Health Assistive Devices Program (ADP) may be completed on behalf of the client if they are assessed to be eligible for funding.

·         File review, communication with the client and or caregiver to determine current mobility status, location of walker use and liaison with client’s vendor of choice to request trial equipment for the assessment

·         Assessment of the client within their home environment using the mobility device to ensure proper fit and that it meets the client’s functional mobility needs, education and training of safe use of the walker will be provided

·         Complete full report of assessment findings and recommendations. If applicable, the Occupational Therapist who is a registered ADP authorizer will complete the appropriate application for funding assistance.

·         Completion of a follow up visit with the client may be required if the walker is not able to remain in the client’s home following the initial assessment visit. In this case, a follow up visit will be required to re-iterate education and training of safe use of the walker.

·         Additional liaison with client’s vendor of choice regarding equipment needs for trial and additional assessment visit.

·         Travel and mileage in addition to the above noted breakdown.
