Occupational Therapy Functional Educational Issues Assessment
 

 

 


The purpose of this assessment is to thoroughly review the client’s physical, cognitive and affective accident related injuries, impairments and sequelae for the purpose of determining the impact they have on the client’s ability to perform their activities of normal living related to completion of educational activities.

 

As well, this assessment addresses the fit between the client and the physical and social environment of his/her school.  In addition, a review of the client’s school environment and or home study area will be conducted to address safety and ergonomic concerns.

 

Following the assessment, recommendations will be made to assist the client in reducing their symptom exacerbations while at school. Recommendations will also be made to minimize the impact of the ongoing injuries, impairments and sequelae and to ensure optimal comfort, safety, and productivity with respect to participation in educational pursuits.

 

This assessment will entail both qualitative and quantitative methods; including completion of client self-report documents, a clinical interview, physical, cognitive and affective screening protocols and functional assessment and observation of tasks and activities in which the client presently has deficits. A medical chart review will be completed if available.

In order to complete the Educational Issues Assessment, the therapist will;
