
The assessment report is based on meetings and functional assessment/observation of the client, an analysis of previous education, work history and current functional limitations. Additionally the information obtained by the available medical documents, clinical notes, standardized testing and the client’s self report of their pre-accident and post accident level of function and interests have been reviewed to assist with this process.
Communication with the client (set up assessment visits)

Three in-office meetings with the client to include;
Visit A: Initial assessment of client’ physical, functional and cognitive abilities and administration of Canadian Work Preference Inventory
Visit B: Discussion of work/educational history as well as discussion of possible job matches, labour market information, wage information, CWPI results
Visit C: Determination of recommendations regarding client’s job preferences, guidance through vocational exploration, retraining opportunities (if warranted)
Research regarding job matches with physical and cognitive abilities and labour market information and implementation of TSA tools (utilizing Profile 2004)
Scoring of Standardized Assessments and Data Analysis
