At Maximum Independence Inc., we attempt to provide very comprehensive and client centered assessment and treatment services.
We strive towards offering services in a timely manner to assist our clients in progressing as quickly as possible towards achievement of their treatment goals.

In order to expedite the referral process we have implemented an online referral system. Two separate and distinct referral forms have been made available for our clients who have access to Accident Benefits and for those who are requesting Medical Legal Services.

Please note that our referral system and forms are encrypted to ensure that all information sent to our offices is secure and private. For your added convenience, you may now attach computerized medical information directly with your referral, or alternately feel free to send it to our office separately via fax, mail or courier. With the changes to the rules of Civil Procedures a specific referral letter and associated questions list are required for all medical legal requests.
